*Please note, filling out these forms DOES NOT officially surrender your dog(s) to Colorado Puppy Rescue.*

*We will review the information and contact you to discuss options.*

Colorado Puppy Rescue is 100% foster-based. We do not have a facility or shelter where dogs stay. All dogs are kept in foster homes until they are adopted so we are unable to take an owner surrender without scheduling in advance.

Please call your vet to get all records to surrender with the dog.
